Liability

I, named above, hereby release Wayne Harding from any liability or claims that could be made against (him/her) concerning my mental and/or physical well-being during the work that has been outlined and agreed upon (now and in the future) by filling out this form.

Scope of Practice

I understand that Wayne Harding is not a licensed physician, psychologist, or medical practitioner of any kind and that Wellness Coaching should not be considered a replacement for the advice and/or services, of a psychiatrist, psychologist, psychotherapist, or doctor. Wayne Harding does not prescribe, or recommend changing dosage or discontinuing, any prescription medications or pharmaceutical drugs.

Guarantee

I understand that although Bioresonance Scanning has an incredibly high success rate, Wayne Harding cannot and does not guarantee results since my own personal success depends on many factors that Wayne Harding has no control over, including my willingness and desire to affect the Lifestyle changes for myself.

Audio Recording(s)

Ideally Wayne Harding would not like any sessions recorded. However, he understands that if a recording (or recordings) are made during or after the session(s), he retains full copyright over any forms of media that may be produced or distributed publicly or privately in any other media outlets or social media platforms.

Confidentiality

By signing this form, I understand that Wayne Harding may discuss aspects of my case with other qualified herbalists and a wholistic Doctor on his panel of specialists. He will always keep my full name and identity completely confidential unless I have given permission otherwise.

Charges

Wayne Harding charges a fee of £150 for a 90 minute consultation which includes using one or more of his Bioresonance machines. This also includes 4 months of back-up support if you follow his recommendations. Rescans are done only after 4 months at a cost of £75 after following his recommendations only. A questionnaire may be included as a way of fully assessing your needs, which will assist him in composing your evaluation report. Discounts may be offered on referrals.
